NEX-Forms Pro is a visual form builder for WordPress. Drag and drop more than 30 form elements into a grid layout, which supports unlimited nesting. Bootstrap-styled fields, styling controls, and a live preview help you arrange forms and check their responsive layout as you work.

Field types support various collection tasks

Build contact and support forms, surveys, polls, quizzes, questionnaires, applications, bookings, orders, subscriptions, cost estimates, and payment forms. Available fields include text and selection controls, ratings, date and time pickers, and file and image uploads.

Answer rules guide users through longer forms

Multi-step forms divide questionnaires, bookings, and quizzes into stages with progress indicators. Conversational forms can present one question at a time, while interactive forms use live input to make a conversational flow. Conditional logic can display or hide fields, sections, or steps according to a user's selections and submitted values.

Math logic calculates values from user selections and entered information, which can support estimates, orders, and service requests. Booking forms can collect dates and times with picker fields. Quiz forms can show different messages or outcomes based on scores.

Display options put forms in the right place

Embed a form in page or post content or a sidebar, or present it in a modal popup, sliding form, or sticky side tab. Visitors can open forms from buttons, links, or other page elements. Automatic displays can use a time delay, a scroll position, or an exit-intent trigger. More than 70 animations and a selection of icons provide additional styling options.

Submission settings handle follow-up and payments

File upload fields collect documents and other documents, with file type validation. Autoresponders, administrator notifications, email settings, confirmation messages, and built-in anti-spam controls help manage submissions and follow-up. Form entries can be stored and exported. PayPal integration supports payment forms, order forms, and booking forms that receive payments through PayPal.

Form setup connects layout, behavior, and display

In the form builder, start a new form and add fields with drag and drop. Arrange them in the grid, then configure any multi-step layout, calculations, conditional rules, notifications, or confirmation settings the form needs. Choose a display method, preview the responsive layout, and embed the form in page or post content or configure its popup or triggered display.
