Choose a form workflow

Forminator Pro uses a visual drag-and-drop builder for contact, order, payment, email, feedback, application, and registration forms. You can also build polls, quizzes, and calculations. Pro unlocks all form fields and premium add-ons from the start, so you can configure fields for the task and add the finished entry to your WordPress site.

Collect opinions, answers, and inputs

Polls collect user opinions with configurable settings and can display real-time results. Quizzes can test knowledge or use a no-wrong-answer format, with shareable results and an optional lead form to collect participant details.

Calculation forms use visitor input to collect information, generate leads, take orders, or return tailored results. Examples include product orders with size and color variations, BMI or calorie calculators, loan calculators, service estimators, and return-on-investment calculators.

Set up payments and subscriptions

Stripe and PayPal integrations support fixed or variable payments for purchases, donations, down payments, and merchandise. Enter the relevant publish keys to activate the payment module. Stripe’s Dynamic Payment Element can offer more than 40 payment methods, including Apple Pay, Google Pay, and Klarna, with controls for which options visitors see.

Forminator Pro includes Stripe subscription forms. Configure selectable plans or pricing, billing cycles, custom upsells, and an optional free trial. Subscription form data can sync with the connected Stripe account.

Add Pro tools for signatures and documents

The e-signature field lets visitors sign with a mouse, trackpad, or finger on a touch device. Forminator Pro also includes dynamic PDF generation and geolocation with autocomplete for workflows involving documents or location input.

Add the configured form to your site

Choose a form, poll, quiz, calculation, or payment workflow, then add fields and set its options in the visual builder. Configure Stripe or PayPal publish keys for payments, or select the Pro tools needed for subscriptions, signatures, PDFs, or location input. Add the finished item to the relevant WordPress page or content area.
